Once you complete the form, PRINT the form and SIGN (if required).  Then submit to the Township by:

Mail:  1145 Garver Lane, Chambersburg, PA 17202

Office:  Drop off at the Township office between the hours of 8:00 am – 4:00 pm, Monday thru Friday

Drop Box:  Located outside the Township office front doors.
